BAND FORMS!
Required Band Forms: 2 things to complete!
- 1 form for the band
- An online packet of forms for the district
Band Student/Parent Info and Band Dues/Expenses Form
- Can be completed online:: https://bvnbandboosters.boosterhub.com/store/1416 (Student info button)
- Or can be submitted in Hard Copy. CLICK HERE for a downloadable PDF.
- Dues Payments payable to BVN Band. Check or online payments available.
- Due date: Marching Mustangs please complete by/before your uniform fitting at Band Camp. If submitting in hard copy please bring the form and dues check with you to your uniform fitting! If completing online, please complete prior to your uniform fitting.
- Due Date: Non-Marching Mustangs: please complete and submit by the 2nd Friday of School in August! If submitting hard copy, please turn it into the Blue Box in the Band Room, just outside Mr. Freeman/Mr. McCarty’s Office.
Required BV District Forms
- ALL Online. Please complete here: https://bluevalleysd-ar.rschooltoday.com/home
- Please complete the following: Health Form – Transportation form – Media Form – REQUIRED for ALL Band Students
- For Marching Mustangs Only – please also submit your Physical form at the link above. Physical must be completed after May 1 2023 and on file by Monday, August 7th, 2023
- These forms will transfer to all District activities/clubs/teams!
